Serious Health Condition - Condition Causing Incapacity

Does the health condition require the individual to be under the continuing supervision of a health care provider even though treatments may not be effective?

(Note: The individual may be under the continuing supervision of a health care provider even if he or she is not receiving active treatment. Examples of this type of condition include Alzheimer's disease, a severe stroke or the terminal stages of a disease.)
