Military Family Leave Certifications

* NOTE: If you have not already established that the military family leave provisions apply to your situation, we strongly advise you to return to Military Family Leave Introduction and answer the questions for Qualifying Exigency Leave or Military Caregiver Leave before reviewing certification.
An employer may require that an employee’s leave for a qualifying exigency or to care for a covered servicemember with a serious injury or illness be supported by a certification. An employer must give notice (written or oral, as applicable) of this requirement for certification each time it applies.
